“…Dentro de las recomendaciones más destacadas se encuentran las del manejo de la presión intracraneal, de suma importancia, ya que una vez que las lesiones ocupativas aumentan de volumen, la complianza intracraneal disminuye, hasta que se supera la capacidad reguladora, cuando las lesiones ocupativas se expanden causando lesión neuronal, síndromes de herniación y muerte encefálica.13 El manejo inicial del paciente con TCE debe cubrir los siguientes puntos, que nos ayudarán a garantizar la supervivencia neuronal después de una lesión cerebral traumática grave: ● Temperatura: se debe evitar la hipertermia en pacientes que presente TCE grave ya que aumenta los niveles de aminoácidos excitatorios y radicales libres de oxígeno, favorece a la inhibición de las enzimas proteolíticas, ruptura de la barrera hematoencefálica y aumento del área isquémica en regiones vulnerables, también favorece a la hipoxia cerebral debido al aumento del metabolismo. …”
