| description | Evaluar la adherencia a hipoglucemiantes orales en pacientes diabéticos tipo 2 de la Unidad Médica Familiar (UMF) 24 de Cintalapa Chiapas del Instituto Mexicano del Seguro Social (IMSS). En el 2009 se realizó un estudio transversal analítico, con un registro de 259 diabéticos de los cuales fueron elegidos 100 de manera aleatoria consecutiva en la consulta de medicina familiar; mediante el cuestionario Morisky-Green se determinó la adherencia al tratamiento asimismo se midieron variables sociodemográficas y clínicas. Hasta el 80% de los pacientes fueron adherentes. Los diabéticos con glucemias mayores de 126mg/dL presentaron 4.4 veces más riesgo (OR 4.4, IC 95% 1.26-15.11) de no adherirse al tratamiento. La mediana de comprimidos fue 4 (DE ±2.3), mientras que las cifras de glucemia de 139 mg/dL(DE ±60.3mg/dL). Los pacientes que no se les facilita acudir a la consulta de control tuvieron 5.4 veces más riesgo (OR 5.4, IC 95% 1.8-14.4) de no ser adherentes, mientras los que consideran existe lejanía entre la UMF y su casa tuvieron 5.1 más riesgo (OR 5.1, IC 95% 1.814.2) Se debe realizar de manera rutinaria la detección de pacientes no adherentes al tratamiento farmacológico, considerando la accesibilidad a la consulta médica y la relación médico-paciente. AbstractObjective: To determinate the adherence to oral hypoglucemiants in patients with diabetes type 2 from de Unidad Médica Familiar (UMF) 24 of the Instituto Mexicano del Seguro Social (IMSS). Material and methods: During 2009 a transversal analytic study was made with a record of 259 patients with diabetes type 2 of those who where randomly chosen 100 patients in the family consult, trough the Morisky-Green test was determined treatment adherence and sociodemographic and clinic variables were investigated. Results: Until the 80% of patients were adherent. Diabetics with blood glucose of 126mg/dL or more had 4.4 more risk (OR 4.4 IC 95% 1.26-15.11) of being non adherent. The median of tablets was 4 (SD ±2.3) per day, while blood glucose was 139mg/dL(SD ±60.3mg/dL). Patients that considered they had not ease for an appointment had 5.4 more risk (OR 5.4, IC 95% 1.8-14.4) to being non adherent and those who considered a far distance between the UMF to their houses had 5.1 more risk (OR 5.1, IC 95% 1.8-14.4). Conclusion: We most make a regular screening for detection of non adherence to treatment considering accessibility to medical consult and the relationship between doctor and patients.
